Field Trip Permission Form

Students at Heilicher take several educational field trips throughout the school year.

By signing below, you will be giving your child permission to attend all field trips for his/her class during the 2017-2018 school year.

Before each trip, you will receive the details including the destination, purpose of the trip, transportation plan, materials needed, and time away from the school building. If, after reviewing this information, you prefer that your child not attend a particular field trip, you may always let your child’s teacher know by email. A tentative list of field trips by grade appears below.

Please complete the following form and submit it by September 1, 2017.

If you have any questions, please contact your child's teacher/advisor.
